For crispy, craggy edges, tear the bread into pieces.
For the best golden brown, crunchy croutons, bake them on a rimmed metal pan, not glass or ceramic bakeware.
Baking time depends on the dryness of the bread. The drier the bread, the shorter the baking time.
If needed, bake them at a higher temperature alongside other things, up to 425 degrees. Just leave out the Parmesan -- it can burn above 350 -- and reduce the baking time.
